Fill in the top box with the estimated number of minutes you spend on each activity. Fill in the bottom box with the number of minutes your family spends on each activity. Type a 0 for an activity that you or your family does not participate in.

Did you know?
- Only 5 percent of Americans do vigorous activity on a given day. (Tudor-Locke, Johnson, Katzman, 2010)
- Seventy percent of Americans do not get the amount of physical activity they need.
- Two percent of children age 6-11 years get the recommended 60 minutes a day of physical activity.
- Eight percent of adolescents get the recommended 60 minutes a day of physical activity.
- Five percent of adults get the recommended 30 minutes a day of physical activity. (Troiano, Berrigan, Dodd, Mâsse, Tilert , McDowell, 2010)

Family Activity Chart (download the chart below)
- Print the Family Activity Chart and place it where everyone in the family can participate (the refrigerator is great!).
- Each time a family member completes 15 minutes of physical activity they write their name on a square. (Up to two per day.)
- When two or more family members complete 15 minutes of physical activity together, put an “X” on a “Family Time” square.
- Award non-food prizes for individuals each time they complete five. Plan a special family outing or activity when five “Family Time” squares are completed.
